Go to the CNB Internet Portal and look for e-services. Click on e-Notif to access the portal. You would need to log in using your Singpass account.
You need to be registered under the SMC in order to access the e-Notification landing page.
Please try the following two steps.
a. Clear your browser cache under your ‘Settings’ and restart your browser again to access the e-Notification website.
b. Access the e-Notification website via Incognito / Private browsing mode on your browser.
On the e-Notification landing page, click on ‘Submit New e-Notification’ to create a new medical notification. Fill in the relevant details and your observations in the ‘Patient and Consultation Detail’ page. If the patient has a past record, enter the Patient ID Number. If you have submitted an e-Notification before, your healthcare institution and the relevant address will be populated. Otherwise, select your healthcare institution from the dropdown list.
Once you have completed all the fields, click the ‘Submit’ button to submit the medicalnotification letter. We would then process your submission accordingly.
Yes, click ‘Save’ to save your medical notification letter as a draft before submitting. Your draft will only be saved for 7 days. After 7 days, your draft will be removed.
After logging in with your Singpass account, in the e-Notification landing page you can search for your submitted medical notification letters using the search parameters. Click on the ‘View’ button to view a submitted medical notification letter.
In the e-Notification landing page, you can search for your draft medical notification letters using the search parameters. Click on the ‘Edit’ button to edit your draft medical notification letter.
Your previously populated medical notification letter will be displayed, and you can update the fields accordingly.
After the necessary amendments are performed, submit the medicalnotification letter by clicking ‘Submit’. We would then process your submission accordingly.
If you click on the ‘Cancel’ button during the creation of a new medical notification letter, your medical notification letter will be voided. Any unsaved data will be lost. A pop-up will be displayed for you to confirm if you would like to leave the page.
No, once submitted you are unable to amend or delete the medical notification letter.
Click on the ‘Print’ icon to generate a PDF copy of your medical notification letter. This can be downloaded and printed.
After submitting your first medical notification, you will be able to submit another medical notification for the same patient, by clicking the ‘Submit More e-Notification’ button on the acknowledgement page. Select ‘For the Same Patient’ option and a new medical notification will be created with the same details as those you just submitted.
Selecting the ‘For a Different Patient’ option allows you to create a new medical notification for another patient.
